With a tax credit building, there is more paperwork to complete than the average assisted living. We have to complete all forms that MN Housing mandates. Luckily, we have staff on-site that are able to assist and walk you through the paperwork.
Firstly, you want to ensure your loved ones has their finances in order. If they are unable to privately pay for nursing services each month, you will need to start the process of applying for the waiver. You can do this online at MN Department of Human Services. Secondly, we will need to complete a nursing assessment to see if we can meet your loved ones needs. Once approved, we will gather all financial proofs. Once completely qualified and approved, we can plan a lease date and pick out an apartment.
No, at The Sanctuary at Brooklyn Center your loved one can move in with an active waiver or transition onto one at any time. Keep in mind, the assets do need to be under $3,000.00 to be eligible for the Elderly Waiver.

Yes, The Sanctuary at Brooklyn Center has a medication management program, which may be added to an individualized service plan and varies based on regulations. Residents participating in The Sanctuary at Brooklyn Center’s medication program may choose their own pharmacy or use Fairview preferred pharmacy provider in that region. If a resident prefers to use an alternate pharmacy, the resident and pharmacy must comply with certain requirements.
Upon move-in, our nurses and care team will assess your current health and review your medical history to see how we can best meet your needs. They will also discuss your preferences as well as your normal routines to develop a care plan that’s tailor made for you. This individualized service plan details your care plan and is regularly updated to meet your evolving needs.
We provide continued education and training, clinical care protocols and multiple quality assurance measures to reinforce our strong standards. The Sanctuary at Brooklyn Center has extensive training programs have been developed throughout the years and include; classroom, online and hands-on training, and employee shadowing and mentoring.

Our base rate in assisted living and memory care includes three meals a day, housekeeping, a load of personal laundry and fresh linens and towels each week, an emergency safety alert system, wellness checks, transportation to community programs, and all of our in-house activities.

In assisted living, we serve three meals a day and our dining services coordinators serve our residents restaurant-style. Emphasizing choice is important. In addition to the unique entrees featured at each meal, we also offer an “always available” menu for our residents.
As this is our residents' home, we welcome visitors and work with our family and friends to visit their loved ones at a time that works best for them.
Yes, we accept companion animals. You do have to independently be able to care for the animal. Guidelines apply.
We work with each family to support their needs as best we can, including accommodating urgent move-ins when possible. Residents in Assisted living, Memory Care receive a full care assessment prior to moving in, which helps our care team develop his or her Individualized Plan. In some cases, a move-in can occur in as little as a day or so.
